Nuclear Stress Test

A nuclear stress test combines two diagnostic techniques — a stress test and nuclear imaging — to reveal problems with your heart.

The test begins with nuclear imaging showing heart activity at rest. Then, you will undergo a stress test to raise your heart rate. After finishing the stress test, your provider takes a second image while your heart rate is elevated.

Comparing the before and after images shows the changes in blood flow and how well your heart functions when it’s forced to pump more blood to your body using

EKG

An EKG shows your heart’s electrical activity. EKGs provide quick information, making them the first diagnostic test performed to find signs of a heart attack, detect arrhythmias (an irregular heartbeat), and find signs of cardiovascular disease.

ICD

ICD's are an implantable device that monitors your heart and send an electrical pulse when needed. It detect rapid or chaotic heartbeats that often cause a sudden cardiac arrest. When your heart stops because of an irregular heart rhythm it provides and electrical shock (defibrillate) so the heart restarts and prevents sudden cardiac arrest.  It can save your life. It is also equipped with a pacemaker feature that restores a slow heartbeat.

Holter Monitoring

Holter monitoring, also called ambulatory electrocardiographic monitoring. It is a diagnostic test that records your heartbeat as you go about your normal daily activities.

You wear the battery-operated Holter monitor for about 24-48 hours. The device continuously picks up and records your heart’s electrical activity, just like your in-office electrocardiogram (EKG).

Echocardiogram

An echocardiogram is a diagnostic cardiac ultrasound that shows moving image of the structures in your heart. It uses high-frequency sound waves to create these images.

A highly trained technician performs your echocardiogram using a probe called a transducer.  The transducer emits sound waves that painlessly penetrate your skin and muscles to reach your heart. You can't hear the sound waves because they're too high of a pitch for human ears.

The sound waves hit your heart and bounce back to the transducer, which relays the information to the computer system. The computer's software translates the echoing sound waves to form images of the valves, chambers, and blood vessels in your heart.

Chronic Care Management

Once chronic diseases develop, they can’t be cured. But you can still take steps to protect your health and enjoy an active life with help from chronic care management. Chronic care management provides the medical treatment you need to keep your body healthy, reduce or eliminate your symptoms, and stop the disease from getting worse.

What conditions need chronic care management?

These are only a few examples of chronic diseases that need ongoing management:

  • Heart disease

  • High blood pressure

  • High cholesterol

  • Diabetes

  • Arthritis

  • Asthma

  • Chronic kidney disease

  • Chronic obstructive pulmonary disease (COPD)

All of the conditions listed heart disease (including asthma and COPD) directly contribute to cardiovascular conditions, potentially leading to heart attacks and strokes.
